Jak poprawnie zapisać formularz?

Zbudowałam próbny formularz, w którym jest 10 pytań. Nie wiem jak go zapisać. W podglądzie nie ma żadnego z pytań zawartych w formularzu. Podobnie, gdy go udostępniam jest pusty. Co robię źle?
