Now, coming back to your question, this can achieved by adding two Date fields in your form and a Form Calculation Widget. Check out the steps and screencast below to see how it's done:

1. In Form Builder, click on Add Form Element + and on the left panel look for the Date Picker Field. Add two Date Picker fields to your form.
   
   
2. Now, click the first Date Picker field and click the Gear Icon. In the right panel, go to Options Tab and set the Default Date as Current. Then go to Advanced tab and Turn on Read Only slider.
   
   
3. Next, click the second Date Picker Field and click the Gear Icon. In the right panel, go to Limits tab and Uncheck Future checkbox to allow form fillers to enter past dates only as this is a birthday field.
   
   
4. Next, add the Form Calculation widget to perform calculations for age.
   
5. Finally,set the calculation for the Form Calculation widget, as shown below:
