Mental Capacity Assessment Form
A mental capacity assessment form is used by medical doctors or healthcare professionals to determine whether or not a patient has the mental capacity to make their own healthcare decisions. The person filling out the form may be any healthcare professional, including a registered nurse, physician assistant, social worker, therapist, psychiatrist, or psychologist.
This form may be used in any number of situations, including when someone is unable to make their own healthcare decisions for themselves for any reason, including whether or not they are competent, experiencing cognitive decline, experiencing mental illness, experiencing dementia, or experiencing any other form of cognitive impairment.
