Community Shift Report is a tool that is used to record things that happened during the community shift of the staff. In this form, all activities, observations, and results are documented in a way that is easier to read and understand. Because this report will be read by the next staff who will replace your shift or the staff on the following shift.

This Community Shift Report contains form fields that ask for the staff information and shift details. This form template is using the Date Picker tool in order to identify the shift start date and end date of the staff. This form template is using the Show and Hide Field Condition wherein certain fields can be hidden based on the answer selected by the respondent from another field.
