I created a form, went to create the qr code and it started going really slow. Once I got the qr, I scan it and it just says slow upstream on a white screen. Every time I try to log back in now it tells me to check my internet connection, which is just fine. Please advise.

I am trying to set my form to where the same person can’t vote twice, but when I select the option under form settings for Unique submission, it doesn’t allow more than one employee vote, it tells everyone else using there work computer to vote that they have already voted. Is there a different way to set this form to where only one vote per person?
