Medical Form with Interactive Pain Area Marking

    Hi

    I am looking to create a patient intake form in which I would have an image of a human body in which I want to allow patient to fill out digitally on computer or on iPad.

    Is it possible to display a human body in which patient can use the mouse to circle pain points on the body image, or if they are on an iPad use their figure to highlight or circle the pain areas on the image?

    I am enclosing a form that I am hoping to digitize and the area showing a human body that allows for circling on the diagram is what I am looking to reproduce in jotform is this possible?

    Hi Jerry,

    Thanks for reaching out to Jotform Support. Yes, it is possible to create a patient intake form with an interactive human body diagram in your form by using the Draw on Image widget. You can allow patients to circle pain points on the body image using their mouse on a computer or their finger on an iPad.

    Let me show you how:

    1. In Form Builder, click on the Add Element menu on the left side of the page.
    2. Under the Widgets tab, search for the Draw on Image widget, and click on it or drag and drop it to your form.
    3. Upload your image in the Background Image section on the right side of the page.
