How can I build my form?

Im searching for an online form builder where customers can visit the website, fill out a form (with drop down options and typing options), where people can upload many pictures, and acknowledge the disclaimer, photo use agreement, photo release waiver, and submission.

Thanks for reaching out to Jotform Support. Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:

1. On your My Forms page, click on Create Form on the top-left side of the page.
2. Choose Start From Scratch, and select Classic Form to create an empty form, or Use Template to choose among our 10,000+ ready-to-go templates (Start From Scratch was used in this example).
3. Now, in Form Builder, click on the Add Form Element menu on the left side of the page to see the complete list of elements and widgets.
4. Under the Basic, Payments, or Widgets tabs, scroll down the list, and then drag and drop the fields onto your form. And that’s it.
