Issues with Jotform Integration on Shopify Checkout Page

I am having trouble with my JotForm account populating in my shopify website checkout page. My uploader script is not working?

I've had a JotForm account for about 4 years now and this is the first I've had trouble.

Thanks for reaching out to Jotform Support. Is it possible for you to re-embed the form to your website, if so, could you try re-embedding the form on your website? If you can, let me know if you've done that. Also, could you provide more details about the issue? For example, are you experiencing any error messages?

I've cleared your form cache, as old cache and cookies can sometimes cause these issues. If possible, could you take a screenshot or record a short video of what’s happening? This will help us see and understand the issue better. If we can't resolve it here, I’ll create a performance ticket and forward your screenshot or video to our developers for further investigation. You can share the video on a public platform like Google Drive or Dropbox, or use Loom to record it and send us the link.

In the meantime, let me show you how to post a screenshot to our Support Forum:

1. On the Support Forum page, scroll down to the Your Answer section and click on the Image icon.

2. Drag and drop your image into the Upload box, or click on it and select your file.

3. Then, click on the Add button in the bottom-right corner of the window.

4. To resize it, click on your screenshot and then click on one of the boxes in the corners, and drag it inward.

5. Once you're finished, click on the Post Answer button at the bottom right of the Your Answer section.
