Can I have an external form and only use Jotform form collecting payments?

I have this form - see copy attached - and would like to link this to the payment side of JotForm.

Clicking the "Make Payment" button should take me to your payment page.

I'm using html and PHP.

Thanks for reaching out to Jotform Support. Unfortunately, it's not possible to have an external form and use our Payment Integrations to accept payments. You can re-build your form using our platform instead. The payment integration steps vary based on your selected Payment Processor. But to give you an idea, you can check out this guide about creating your first order form.
