Now, coming back to your question, I can see that the form field you're referring to is a Single Choice field. It's currently not possible to add an image to this element. As a workaround, we can use either the Image element or the Paragraph element, where you can upload your image to, then place it before the Single Choice question page, since you're using a Card Form layout. Let me walk you through it:

1. In Form Builder, click on Add Element in the upper-left corner of the page, and select Image under the Basic tab
2. On the Properties Settings panel that opens on the right, click on the Upload File button under the Image section.
3. Select the image from your device to upload it.
4. Add the field label to inform form fillers that the image is related to the question on the next page.
