I have checked the form you provided, and it seems like it is currently on a Jotform account with an Outlook email address which is c*n**.*al**@o**l*ok.com. Do you happen to remember an account you have that is tied to an Outlook email address instead of a Gmail one? If you do, you can log in to that Jotform account with an Outlook email address, and you will be able to access your form again.
