We often have issues re-enabling previously deactivated forms. Even though we go to the settings tab and swap from disabled to enabled, upon refresh it swaps back to disabled.

I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
One thing to keep in mind is that Jotform accounts also have active form limits based on the subscription plan. An active form is any form that is currently enabled and available for submissions. With your current subscription, you can have up to 100 active forms.
If you are already at the 100-form limit, you may experience issues when trying to reactivate or create additional forms since the account has reached its active form limit. We also have a guide here that you can check to learn more about Understanding Your Jotform Account Usage and Limits.
