David_Hudson_dhudsonForm ID: 251308134477052 Form title: APFPM Professional Accreditation Application
Issue: There is an orphaned/ghost required field rendering directly above the Submit button. Its label is "I confirm that the information provided in this application is accurate and complete." It appears on the live form and in Preview with a required asterisk, and it blocks every submission with an "Incomplete Values" error ("I confirm that the information provided in this application is accurate and complete. is required"). However, this field does not appear as a selectable or editable element anywhere in the form builder canvas, so I cannot delete or modify it through the normal interface.
What I need: Please locate and delete this orphaned field by its question ID (qid) at the backend.
Critical constraints:
- Do not change the form ID. This form is part of a controlled quality management system and the ID is referenced in our documentation — a clone or new ID is not acceptable.
- Do not affect the 106 existing submissions.
- Leave all other fields intact, including the standard confirmation checkbox if one is present.
Already tried (please skip these):
- Confirmed there is no conditional logic on the form.
- Confirmed the Fee Agreement and Date fields are not required.
- The field cannot be selected or deleted in the builder canvas — it only renders in Preview/live, not as an editable tile.
- Issue persists in an incognito/private browser window, so it is not a caching problem.
- A duplicate confirmation field situation was found; the visible/editable checkbox was already removed, but this orphaned one remains.
Please remove the orphaned field and confirm once done. Thank you.
