I cloned your form to test it, but I couldn't replicate the behavior you reported. The Original PDF was successfully attached to the notification email I received. I've included a screenshot for reference.

Since you are using the default sender email noreply@jotform.com, I suggest setting up an SMTP for your form. I also recommend checking your email provider's documentation for their specific attachment size limits. If that suggestion does not work for you, do we have your permission to test your live form directly? We will use Jotform Test in the fields so you can quickly identify and delete our entry from the table or inbox.
Once we hear back from you, we'll be able to move forward with the solution.

Please try downloading the original PDF from the "Submission Date May 11, 2026 5:10 PM" document in the 貸与品事故報告書 - Jotform Inbox. Also, I have changed the SMTP settings.
