Scrambled questions when downloading the fillable PDF copy of the form

You're incorrect:

Even after recreating my form as a Smart PDF then trying to download it as a PDF, my questions are scrambled. You told me if I recreated it as a Smart PDF that my questions would NOT scramble but they do.

How can I download my form as a PDF without the questions scrambling.
Please don't repeat what you told me to do before because it doesn't work.

Thanks for reaching out to Jotform Support. I cloned your form to test it, and I easily replicated the issue. There is an easy fix for this, we just need to delete the PDF Form and create a new one in PDF Editor. Let me show you how:

1. Open your form in Form Builder, then go to PDF Editor by clicking the Dropdown icon next to the Jotform logo and selecting PDF Editor.
   
2. In PDF Editor, click the Three dots next to PDF Form, then select Delete.
   
3. Click Yes, Delete on the pop-up window to confirm.
   
4. Once it is deleted, click on the +New PDF at the top.
5. Click Create PDF Form.
   
6. Select PDF Version of Your Form.
   
7. Lastly, click on Continue on the pop-up window.
   

That's it. You can now download the fillable PDF copy your form with correct question order. Here's the result:
