hello,
I have several surveys going on now. One of my study particpants informed me that they could choose multiple answers, and I wanted it to be singe answer, so I redid the surveys so they would only allow one answer, but the problem is that now it appears I have lost all the responses from the other forms. I have had about 12 responses for the following forms:
perceived stress, professional quality of life and copenhagen burout.
it is too complicated for me to revert back and capture the prevous reponses, can you please help me wiht that?
can you give me an excel sheet of the responses that I have lost?

Regarding your issue, when you changed the question type from multiple choice to single choice, the previous responses might not display properly. Unfortunately, responses linked to the previous question type may not be directly recoverable in the current form view.
However, you can try using the Revision History feature to revert your form to a previous state before the change. This might help you access the earlier responses. You can also export the submissions you currently have to Excel from Jotform Tables.
If reverting the form is too complicated, unfortunately, there is no direct way to generate an Excel sheet of lost responses after changing the question type.

Hi Julie,
To clarify, did you edit your form and change the Multiple Choice element to Single Choice element? Keep in mind that deleting a form element will also delete the data collected on it. There's a pop-up message when you delete a form element saying the data will also be deleted. I've shared a screenshot of the pop-up message for reference:
Can you share the title or the URL of the form this happened? You can easily find your form's URL in Form Builder. Let's walk through this together:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the Quick Share tab, click on Copy Link in the Share With Link section.
You can also try using the Revision History feature that lets you revert your form back to an older version you select. Let's walk through this together to use it:
- In Form Builder, click on the Circular Arrow icon at the top middle of the page to open the Revision History menu.
- In the menu on the right side of the page, select the version you want to restore the form to.
- Now, just click on Revert next to the version you select, and then close the Revision History menu in the top right corner.

I don't want to try the revert back ,it is too confusing. I didn't realize my data would be deleated. Can you revert it and put the responses on a PDF for me and then revert it back for me? These are the forms:

Hi Julie,
I understand you’d like us to help restore and export the deleted responses for your three forms. Unfortunately, Jotform Support can’t directly revert your forms or access a previous version on your behalf, as revisions can only be applied from within the account. Once a form element is deleted, the associated submission data is also permanently removed from your active tables. If the data was deleted due to the field type change, the only way to recover it is by restoring an earlier version of your form using the Revision History feature. This will allow you to temporarily view and export the deleted data to Excel or PDF before returning to your updated version.
You can use the Revision History feature that lets you revert your form to an older version you select. Let me show you how to use it:
- In Form Builder, click on the Circular Arrow icon at the top middle of the page to open the Revision History menu.
- In the menu on the right side of the page, select the version you want to restore the form to.
