Embedded Form Not Showing in Properly

I've embedded two forms into my website, but some of my colleagues are reporting that the forms aren't loading properly, while other colleagues are.

By the way, my operations team is based in China. Could this be related to the network environment? I noticed that different colleagues are using different Wi-Fi networks.

My website is aimed at the US market, so if the form works in the US, that's fine!
