PDF with Fillable Field Won't Upload

    I have a premade form that I would like to upload.

    For now, just the Company Name is a fillable field for text (for testing)

    There are a couple checkboxes beneath that for Organization Type.

    I can't upload the PDF to Jotform. Can you assist?

    I have attached the form.

    The instructions don't say whether I need the fillable fields already created as fillable in a pdf software.

    However, I have tried with and without, and doesn't upload.

    I imported your PDF form and tested it on my side, and everything worked as expected. Check out the screenshot below to see my results:

    • Clear your browser's cache.
    • Try using a different browser or incognito/private mode.

    Meanwhile, here are the steps for converting your PDF to a fillable form:

    1. On your My Workspace page, click on Create on the top-left side of the page.
    2. Select Form from the assets, and then click on Smart PDF Form.
    3. In Smart PDF Forms, click on Upload Document, and then select the PDF you want to convert to a form, or just drag and drop the PDF into the white upload area.

    If some fields aren’t added automatically, you can manually add them to your form, and then match it to the PDF on the right side of the page.

    Thank you. Didn't work using Chrome on 3 different computers in 2
    different USA States.
    It did work on Mozilla.
    Now, it doesn't allow me to use my actual form, it seems. It converts it to
    a jotform style entry?

    I understand that you are seeing your form converted into a Jotform-style entry rather than retaining the original PDF format.

    This behavior is expected when using Jotform's Smart PDF Forms feature. When you upload a PDF, it converts the file into an interactive web form that matches your PDF's fields. This allows users to fill out the form online, and the data will automatically populate your original PDF file. So, the form you see in Jotform is the fillable web version of your PDF, not the PDF itself.

    Since the upload works on Mozilla Firefox, I recommend using that browser for uploading and editing your PDF forms to avoid the issues you encountered on Chrome. Also, make sure to clear your browser cache or try incognito mode if you want to troubleshoot Chrome further.

    If you want to keep the original PDF layout and use it as a fillable form, Jotform's Smart PDF Forms is the right approach, but it will present the form in the Jotform interface for filling. The filled data will then be merged back into your PDF.
