Truncated Contact Form on my Website

I decided to ADD a Checklist Widget to my form, and now it's truncated again on my website. Here is the URL Contact Form.

I checked your The Shulammite Author Magazines form as well as the Adobe Portfolio site where it's embedded, and the Checklist widget is placed exactly as it is on the form. I've shared a screencast of how the form looks below:

Can you explain a bit more about the location of the Checklist widget and where you're looking to place it, so I can get a better idea of what's happening?

Okay, I did check my form in the Form List and for some reason the Checklist was in the wrong position. So I REFRESHED my screen and it's not correct.

I checked the form on my website and its now altogether CORRECT.

Sorry about that.

I fixed it. It was some widget that got placed there. I just needed to REMOVE it and now it looks terrific!!
