Dear TEACHER,
You are cordially invited to participate in this pre- and post-service questionnaire, including Part 1 and Part 2. Part 1 is based on your personal situation. Part 2 is based on your observations of your student’s self-regulation and socio-emotional development. This questionnaire has two parts; please answer all questions and read the instructions carefully. There is no right or wrong answer; participants should select the most appropriate answer.
VOLUNTARY PARTICIPATION / WITHDRAWAL
Participation in this study is entirely voluntary. You may withdraw from the study by informing the project team at any time without any consequences. If you withdraw from the study, the data collected up to your withdrawal will be used unless you request to have them excluded. The Principal Investigator(s) may terminate your participation if they deem it in your best interest. They may also do this if you cannot adhere to the instructions required by the study.
DATA PROTECTION
All information and data collected from this study will be classified as confidential. The Principal Investigators and their research teams and the Human, Artefacts Ethics Sub-Committee (CityU) / Human Research Ethics Committee (EdUHK), and Human Research Ethics Committee (HKU) will have the right to use and retain your data for the purposes and in the manner described in this information sheet. Data will be retained for 5-15 years, and some data will be used to build a child database. Any presentation or reporting of findings will involve only anonymized data; your identity will remain confidential. Personal information identifying you (e.g., your name) will not be kept in the database. Any hard copies of the collected data will be placed in sealed envelopes and lockers. Soft copies will be stored in password-protected files on cloud and hard drives. Only the research team will have access to the keys to the locked cabinets and passwords.
